The Inclusion Matters service has been updated. Full details of the services are below, together with downloadable leaflets to share.

Home from Hospital

We offer a service to any patient due to be discharged from hospital on a Pathway Zero (i.e. the patient is safe to go home), or any patient recently discharged or any patient due to go into hospital, plus, we now cover any inpatient within any hospital in Cornwall and also Derriford.  Presently we have capacity to take referrals, so if you are a professional please use our referral form below.  We do also accept patients making self-referrals.

Community Support

We also offer Community Support for any individual or group of people that have or are looking to benefit the community in some way.  A variety of help can be offered and all work is bespoke and tailored to individual or group remit.  Please use the referral form below and tick in the box for ‘Community Support’, either if you would like to make a self-referral or refer a group or individual with their explicit consent to do so. More information can be found here Inclusion Matters Service – Supporting Community and Grassroot Groups – Cornwall Link.
